New Patients
New patients are always welcome. Our board certified physicians will perform a medical evaluation, review your medical records/test results and discuss your treatment options. We are also happy to coordinate your care with your primary and/or referring physician.
We encourage you to contact our Scheduling Department at 281.332.1515 and arrange your appointment as far in advance as possible. Our Scheduling Coordinator will discuss your physician preference and appointment time options. You will need your insurance card available when you call us. Our physicians may recommend testing depending on your symptoms or diagnoses. Testing can often be performed on the same day as your appointment or a separate day – depending on your preference.

A new patient form is available for you to complete detailing your medical history.
Please complete this form before coming to our office. You may print the new patient form. If you prefer, we would also be happy to mail the form to you.
Please arrive at least 15 minutes early for your appointment, and take along the following items
- Health insurance card(s)
- Referral form (if required by your insurance company)
- A list of your current medications, including any vitamins and/or herbs (be sure to include the dosage and how often you take it)
- A list of your medication allergies
- Completed patient registration form detailing your medical history
- Any required payment
It is your responsibility to fax your medical records to us at 281.332.2525 prior to your appointment. We need pertinent records relating to your cardiac history, including any cineangiogram or x-ray films.
